To do business with Qwest as a Facility-Based Competitive Local Exchange Carrier (CLEC) or Reseller, the Customer Questionnaire must be completed by you or a representative from your organization. Your Qwest Service Manager will assist you with this process.
The Customer Questionnaire contains instructions outlining what sections must be completed before you can begin to order products and/or services.
The on-line Questionnaire, via the web interface, requires the initial Questionnaire to be completed in its entirety, all sections, before it is submitted and accepted. Once the Questionnaire has been completed and accepted, you will have the ability to update, change, or correct specific sections at anytime via the web interface. The Questionnaire instructs you to complete some additional documents within two weeks after submitting the Questionnaire. If you fail to complete these documents, it could delay your ability to order products and/or services as you may not have access to various tools and reports within Qwest.
Upon completion of the additional documents, they should be e-mailed to wholesale.servicesupportteam@qwest.com. Wholesale will provide you an e-mail response that your documents have been received and are in process.
Qwest requires you update your questionnaire(s) on either an "as needed basis" or yearly to reflect any changes within your company that will assist Qwest in providing you service.
The completed questionnaire is used by Qwest to determine the following:
Customer CLEC Questionnaire
If you are a new Facility-Based CLEC, you must complete the Customer CLEC Questionnaire.

If you are a new Reseller, complete the Customer Resale Questionnaire.
Questionnaire Electronic Access
Customer (CLEC) will use the following process for completion of the questionnaire:
Contacts identified in your Interconnection Agreement (ICA) - will be the initial questionnaire registered input individual as identified in the ICA as the notifications contact. This individual will be required to register once with the Questionnaire system. Once the registration is accepted, this individual will have the ability to input and update the questionnaire for their company in Qwest's Questionnaire System.
Registration and Login requires the individual to input their company name (as identified on their ICA contract), e-mail address (as identified on their ICA contract) and Qwest Contract Number. All of the items must validate correctly, or you will not be allowed access to your company's questionnaire, but will be referred to the Qwest Questionnaire Manager for further assistance.
To allow additional employees access to the questionnaire, other than the individual identified on the ICA, you must do one of the following:
With the contacts to be added to your company's questionnaire profile, please be aware you will be responsible for notifying Qwest of individuals that should be removed from the profile.
Qwest's products and services are available where facilities exist throughout Qwest's 14-state local service territory.
Wholesale rates for this product or service, including tariff references and any applicable discounts, are provided in your current Interconnection, Resale, Commercial, or other governing agreement.
If you begin the process by filing out the Customer Questionnaire required sections and submit it and then determine you do not want to continue, you will not be charged for the work already done by Qwest.
