About Washington Contract Terms and Rate Schedules

 

·        The terms and conditions for Qwest services can be found in our Tariffs, Catalogs and Rate Schedules.

 

·        If you receive a tariffed business or residential service, the Tariff governs the provision of your Qwest service.

 

·        Tariffs for Washington regulated services are filed with and approved by the Washington Utilities and Transportation Commission (WUTC) and serve as the contract between Qwest and you for Commission regulated services set out in the tariffs.

 

·        Qwest Catalogs and Rate Schedules contain the terms and conditions for Qwest's formerly tariffed and price listed services and most deregulated services. Formerly tariffed and price listed services remain under Commission oversight for, among other things, customer complaint resolution. Deregulated services are not under the jurisdiction of the WUTC and are identified as deregulated in the Catalog.

 

·        If you receive a formerly tariffed, price listed or deregulated business or residence service pursuant to a signed contract or other agreement with Qwest and it is currently in effect, then its terms will govern the provision of that service along with any applicable terms in the Catalog or Rate Schedule.

 

·        If you receive a formerly tariffed or price listed or deregulated business service not covered by a signed contract or other effective agreement (including expired contracts or term agreements that have not been renewed), the Catalog or Rate Schedule governs the provision of your Qwest service. ·        If you subscribe to a formerly price listed long distance service, separate Rate Schedules govern the provision of your Qwest service, depending on whether you subscribe to Qwest Communications Corporation provided interstate and international long distance, or to Qwest Long Distance Corporation provided intrastate long distance. ·        As a user of any of the services or features in the Catalog or Rate Schedule, you do not need to sign an agreement, but use of Qwest services constitutes acceptance of, and agreement to the terms and conditions in the Catalog or Rate Schedule.

 

·        Qwest may change the terms and conditions of your formerly tariffed, price listed and deregulated services by adding to or making changes to the Catalog, Rate Schedule or separate contract/agreement and giving you reasonable notice of important changes, such as rate increases, instead of filing them with the Commission.
