By checking the above box, you agree to receive a combined bill from Qwest. Your combined bill will include your current Qwest services and all lines on your Verizon Wireless account on a single bill. You understand certain conditions and requirements apply to determine if you are eligible for Qwest combined billing, including but not limited to:
1. Qwest and Verizon Wireless accounts must not have past due balances.
2. Your existing Qwest and Verizon Wireless services must qualify.
3. You must be authorized to make changes to both the Qwest and Verizon Wireless accounts; both accounts must have the same responsible billing party.
You will want to terminate any electronic payment plan set up through your bank or other financial institution once your request has been processed. However, if you have an automatic payment plan set up through Verizon Wireless (rather than through your bank or other financial institution), it will be canceled for you upon enrollment in Qwest Combined Billing.
Upon enrollment, your Verizon Wireless bill cycle start date may change. This means your first Combined Bill may contain a prorated portion of your Verizon Wireless monthly access charges plus the next month's service charge.
Qwest will notify you when your request for a combined bill has been processed. If your request cannot be completed Qwest will contact you for further information.

If you are a new VZW customer within the state of MN, please visit the consent site, www.verizonwireless.com/qwestloa, once your account is active. If your VZW is active now you can visit the consent site at anytime within 90 days.
